The following are due for all players no later than March 21, 2025:

1. Checklist of Requirements with Fee Amounts (print and use to track your progress)

2. Thunder Medical Release and Waiver Form (print and sign)

3. Thunder Consent to Transport Waiver (print and sign)

4. Concussion Awareness Form (print and sign)

5. Physical Exam/Questionnaire (print, take to your doctor, and sign. Grace is given for timing of physical.)

6. Fees (See Checklist of Requirements above for amounts) 

New Athletes Only: The following items are also due no later than March 21, 2025 for each new athlete new to the Thunder:

• Bring the athlete's birth certificate to show

Please check the calendar for tryout dates, times, and location. Players must tryout to earn a spot on a roster. Player fees are only due for players selected for a roster.

The following are due for all players no later than October 19, 2024 for each player:

The South Charlotte Thunder Fall Registration Meeting for players and parents will be held on Saturday, October 21, 2023 at 3:30pm at the Providence Road Church of Christ 4900 Providence Rd, Charlotte, NC 28226

The following are due no later than October 21, 2023 for each player:

• Physical Exam/Questionnaire (PDF) to be completed by the player's doctor
