2. Thunder Medical Release and Waiver Form (print and sign)

3. Thunder Consent to Transport Waiver (print and sign)

4. Concussion Awareness Form (print and sign)

5. Physical Exam/Questionnaire (print, take to your doctor, and sign. Grace is given for timing of physical exam.) (Previous submitted exam valid if within last 12 months.)

6. Fees (See Checklist of Requirements above for amounts.) 

New to Thunder Athletes Only: The following items are also due no later than March 13, 2026 for each new athlete new to the Thunder:
